The TIME framework is a globally recognized scientific tool for systematically assessing and treating chronic wounds. It focuses on four key elements addressed in order to convert the wound from a chronic environment to one that allows healing.
TIME Elements:
- T — Tissue: Remove dead and necrotic tissue. Without debridement, healing is impossible. Methods: sharp scalpel, enzymatic, autolytic (moisture dressings), biological (medical larvae).
- I — Infection/Inflammation: Control infection and chronic inflammation. Take wound culture, use antibacterial dressings (silver), systemic antibiotics if needed.
- M — Moisture: Moisture balance — not dry and not waterlogged. Choose dressings that absorb excess or add moisture. Balanced moist environment heals 3-5 times faster.
- E — Edge: Assess wound edges. Rolled or calloused edges indicate non-progression — may need debridement or reactivation.
Practical Application:
At every visit we assess all four elements and adjust the plan. It's not a fixed system but dynamic, changing as the wound changes.
