Can skin grafting be used to treat large diabetic foot ulcers?

Yes, skin grafting is an effective treatment option for ulcers that have passed the debridement stage and developed healthy granulation tissue but are too large to heal on their own in a reasonable time.

Conditions for Successful Grafting:

  • Clean wound bed: Healthy red granulation tissue without dead tissue or infection.
  • Adequate circulation: Sufficient blood must reach the area to nourish the new graft.
  • Offloading: No mechanical pressure on the graft area.
  • Blood sugar control: HbA1c below 8% ideally.
  • Good nutrition: Adequate protein to support graft healing.

Types of Grafting:

  • Split-thickness graft: Most common — a thin layer is taken from thigh skin and placed on the ulcer.
  • Artificial skin substitutes: Membranes of human or animal collagen that stimulate patient's skin growth.
  • PRP (Platelet-Rich Plasma): Applied with the graft to improve success rates.
