What is the Wagner classification for diabetic foot ulcers?

The Wagner classification is one of the most widely used systems for describing diabetic foot ulcer severity. It helps doctors communicate precisely and determine the appropriate treatment plan for each grade.

Wagner Classification Grades:

  • Grade 0: No ulcer — but the foot is at risk (calluses, deformities, dryness). Prevention and monitoring stage.
  • Grade 1: Superficial ulcer extending only through the skin layer. Does not reach muscles or tendons. Usually responds to conservative treatment (dressings + offloading).
  • Grade 2: Deep ulcer reaching tendons, joint capsule, or bone but without abscess or osteomyelitis. Needs specialized care.
  • Grade 3: Deep ulcer with abscess, osteomyelitis, or tendon infection. Usually needs surgical intervention for infection debridement.
  • Grade 4: Localized gangrene — in a toe or part of the forefoot. Requires limited amputation with vascular treatment.
  • Grade 5: Extensive gangrene affecting the entire foot. Requires major amputation.
