How Coordination Works:
- Detailed medical report: We write a report for every wound — diagnosis, measurements, plan — sent to the patient's treating physician.
- Recommendations for internist: If diabetes is uncontrolled (high HbA1c), we communicate with the internist to adjust treatment.
- Vascular referral: When low ABPI is discovered — we refer with a detailed report and recommendations.
- Surgeon coordination: For cases needing surgical intervention (grafting, flap, amputation) — we prepare the wound pre-surgery and follow up after.
- Nutrition: We refer to the dietitian with wound information and special needs.
- Mental health: We refer patients with depression or anxiety related to chronic wounds.
