Repositioning is the most important preventive measure against pressure ulcers and perhaps the only one that cannot be dispensed with. The golden rule: every 2 hours in bed, every hour in a chair.
Correct Repositioning Technique:
- 30-degree position: Best — turn patient on their side at a 30-degree angle (not 90 degrees which concentrates pressure on the hip). Use pillows for support.
- Lift don't drag: Dragging on sheets causes friction and shear. Use a draw sheet, slide board, or lifter.
- Pillows between knees: When lying on the side, place a pillow between knees and ankles to prevent pressure between them.
- Off the heels: Elevate heels off the mattress with a pillow under the calves — heels are a very dangerous pressure point.
- Head elevation under 30 degrees: More than that increases shear force on the sacrum.
Repositioning Schedule:
Every 2 hours in bed. Every 15-30 minutes pressure relief from seat (wheelchair). Record repositioning times on a chart.
