How are deep pressure ulcers (Stage 3 and 4) treated?

Treating deep pressure ulcers requires a comprehensive, multi-faceted approach — not just wound dressings but addressing all factors preventing healing.

Treatment Pillars:

  • Complete pressure removal: Cannot heal an ulcer under pressure. Alternating mattress + strict repositioning schedule + positioning pillows.
  • Surgical debridement: Remove all dead tissue — by scalpel, autolytic, or enzymatic dressings. Repeated until healthy granulation tissue appears.
  • Advanced dressings: Based on wound stage — foam for absorption, hydrogel for dry wounds, alginate for light bleeding, silver for infection.
  • Infection treatment: Oral or IV antibiotics based on severity. Wound culture to identify bacteria and appropriate antibiotic.
  • Intensive nutrition: High protein (1.5 g/kg daily), vitamin C and zinc, adequate calories — without adequate nutrition the wound won't heal.
  • Moisture management: Treat incontinence — catheter if needed, barrier creams, immediate sheet changes.
