Compression therapy is the first and most important pillar in treating venous ulcers — without it, the ulcer cannot heal even with the best dressings. Evidence shows compression alone heals 40-70% of venous ulcers.
How It Works:
- Compresses superficial veins reducing their diameter, helping valves work better.
- Reduces the high venous pressure causing the ulcer.
- Reduces swelling by pushing fluids from tissues into lymphatic vessels.
- Improves arterial microcirculation.
Compression Types:
- Multi-layer compression bandaging: 4 layers (4LB system) — the gold standard. Provides graduated pressure of 40 mmHg at the ankle.
- Short-stretch bandages: High pressure during movement, low at rest.
- Compression stockings: For maintenance after ulcer healing — class 2 or 3.
Essential Prerequisite:
ABPI must be measured before any compression — ABPI below 0.8 prohibits compression as it may cause arterial ischemia.
