Iron tablet side effects are common but manageable — they're the main reason patients stop treatment prematurely.
Common Side Effects:
- Nausea and stomach pain: Top complaint — occurs in 20-30% of patients. Worse on empty stomach.
- Constipation: Very common — iron slows bowel movement.
- Diarrhea: Less common but occurs in some patients.
- Black stool: Completely normal — unabsorbed iron oxidizes. Don't worry.
- Metallic taste: In the mouth — annoying but harmless.
Tips to Reduce Side Effects:
- Start with half dose for a week then increase gradually.
- Take with a light meal if stomach can't tolerate empty (reduces absorption slightly but better than not taking it).
- Try different forms: Ferrous gluconate is gentler than Ferrous sulfate.
- For constipation: increase fiber and water. Use a gentle laxative.
- Take with orange juice — improves taste and absorption.
