What thyroid tests are needed and what do results mean?

Thyroid tests are simple, accurate, and done with a regular blood draw. Understanding results helps you monitor your condition.

Basic Tests:

  • TSH (Most Important): Pituitary hormone.
    Normal: 0.4-4.0 mIU/L.
    High = hypothyroidism (pituitary screaming: "Work more!").
    Low = hyperthyroidism.
  • Free T4: Main hormone.
    Normal: 0.8-1.8 ng/dL.
    Low with high TSH = confirmed hypothyroidism.
  • Free T3: Active hormone.
    Normal: 2.3-4.2 pg/mL.
    High = hyperthyroidism.

Additional Tests as Needed:

  • Anti-TPO: Antibodies — if elevated = Hashimoto's disease (autoimmune cause of hypothyroidism).
  • Anti-TSI or TRAb: Antibodies — if elevated = Graves' disease.
  • Thyroglobulin: For thyroid cancer monitoring after treatment.

When to Test:

  • Symptoms of hypo- or hyperthyroidism.
  • Family history of thyroid disease.
  • Women planning pregnancy.
  • As part of routine comprehensive checkup.
